The player must prevent pedestrians from falling into one of four sewers by temporarily bridging the open gaps with a manhole cover. If one falls through, the player will lose and have to start over.

In the Super Smash Bros. series, Mr. Game & Watch's down-A attack makes him hit a opponent with the objects he used to cover the ground in Manhole.
- PORTS -
An e-Reader version of the game was sold with every purchase of an e-Reader for free called Manhole-e.
The New Wide Screen version of Manhole was recreated in Game Boy Gallery for the Game Boy in both a close approximation of its original form and in an enhanced version featuring Yoshi, while Mario occasionally runs across the screen. It also appears in Game & Watch Gallery 4 for the Game Boy Advance and as a DSiWare game on August 19, 2009.
